Overview
This investigation reviewed the adequacy of the responses of government agencies to nine electrical incidents that resulted in the deaths of 12 people, including a three-year old child. It also analysed the effectiveness of changes made to Queensland’s electrical safety framework since those incidents occurred. The investigation found poor investigative and regulatory behaviour resulting from poor recruiting practices, inadequacies in training, ineffective investigative procedures, poor recordkeeping practices and lack of effective supervision.
