Overview

This investigation reviewed workplace death investigations undertaken by the Office of Fair and Safe Work Queensland (OFSWQ). The investigation found the OFSWQ was deficient in planning investigations, identifying issues and gathering evidence, and providing legal advice to support prosecution decisions.
