Brooke Brennan Report (2002)

This investigation reviewed the administrative actions of the then Department of Families, Queensland Health and the Queensland Police Service in relation to the safety of the late Brooke Brennan. The report examined the appropriateness of the actions taken and decisions made by the three agencies in their dealings with Brooke and her mother days before Brooke’s murder. It looked particularly at their responses to issues relating to Brooke's safety. The investigation found that communication and coordination problems among the three agencies contributed to the ineffectual response to issues regarding Brooke’s ongoing safety. The report was tabled on 31 May 2002.
